Abstract

Introduction: cryptococcosis is an infectious, systemic and opportunistic mycosis in HIV and AIDS patients. To date, few studies on Cryptococcosis and HIV and AIDS have been done in Angola. Thus, this study was carried out to determine the prevalence and factors associated with cryptococcosis infection in patients with HIV and AIDS at Hospital Esperana, a tertiary hospital, in Luanda, Angola.

Methods: a cross-sectional study was carried out from March 1 to June 30, 2016. The data was obtained through a semi-structured questionnaire administered to participants, and a review of clinical processes and laboratory tests. For the associated factors were using the odds ratio, Chi square test (nominal variables) or Z test (numerical variables) with 95% confidence level.

Results: of the 165 samples (55%) were processed, of which 33% were positive for cryptococcus and 75% were from female patients. The median age of these patients was 38 years, and 38% of the patients were from the city of Luanda. The factors associated with cryptococcal infection were: Cough (OR = 4.00; 95% CI: 2.01 - 7.93; p < 0.001); Edema (OR = 4.53, 95% CI: 1.90-10.80, p = 0.007); CD4 <100 cells/mm3 (OR = 7.81, 95% CI: 3.06-19.20, p = 0.004); TGO> 50 mg / dL (OR = 1.08, 95% CI: 1-175, p < 0.001); Anemia (OR = 6.72, 95% CI: 1.72-25.16, p = 0.004); Pneumonia (OR = 4.21, 95% CI: 1.55-11.44, p = 0.004); Tuberculosis (OR = 3.99, 95% CI: 1.74 - 9.17, p = 0.001) and Malaria (OR = 6.73, 95% CI: 2.73-20.84, p < 0.001).

Conclusion: the present study showed a 33% prevalence of cryptococcosis infection among the HIV and AIDS patients under follow-up in Hospital Esperana, Luanda, which might be indicative of patients presenting to care at a late stage of disease and with low CD4 cell counts. To our knowledge this is the first study that evaluated the prevalence of cryptococcosis in HIV patients in Angola and this data should inform future HIV and AIDS policies and public health strategies.
